Too often, people don’t discover care management until they are in crisis—overwhelmed and unsure where to turn. We know it doesn’t have to be that way. At Connecticut Community Care, we walk alongside individuals and families, helping them anticipate changing needs, build thoughtful plans of care, and live how and where they choose. No one should have to face this journey alone or unprepared.

Every day, the individuals and families we support inspire us with their courage and life stories, while their unique needs motivate us to provide thoughtful support that honors choice, autonomy, and dignity. Equally important are the communities in which our clients live, which shape their experiences and strengthen the connections we foster. Our vision—of a collective community that encourages connection, respects the rights and contributions of individuals of all abilities and ages, and provides the resources to live how and where they choose—guides everything we do.

In a time of uncertainty, our purpose remains clear. CCC’s legacy of excellence is affirmed through the continued renewal of our NCQA (National Committee for Quality Assurance) Case Management accreditation, as well as through the meaningful feedback we receive from the clients and families we support. Our dedicated teams are transforming care management every day through personalized approaches, trusted relationships, and deep community connection.

Sonnet’s Story

Sonnet’s journey is one of resilience, renewal, and joy. After facing a serious health challenge, she has embraced a new chapter in life—one grounded in steady support, meaningful relationships, and moments of genuine happiness.

Central to her story is her brother, whose constant encouragement has helped Sonnet reconnect with a beloved childhood passion: building with Legos. What began as a simple pastime has grown into a powerful source of creativity, fun, and focus—strengthening her hand dexterity and eyesight along the way. Together, they take on new kits, celebrate completed projects, and share laughter and conversation that fill the room with warmth.

Sonnet is also supported by a dedicated care team deeply invested in her well-being. CCC Care Manager Amie, along with Sonnet’s Private Care Assistant through the PCA Waiver Program, brings consistency, kindness, and companionship to her daily life. From assisting with household tasks and meal preparation to offering meaningful connection, their support provides both practical help and peace of mind.

Surrounded by family and caregivers who believe in her, Sonnet is thriving at home. She moves through each day with gratitude, strength, and hope—reflecting the resilience of her journey and the joy of the life she is living today.

CARE MANAGEMENT
Meeting Goals, Advancing Communities

In partnership with the State of Connecticut, we provide in-home care support and transitional support to thousands of people through these programs:

Sage Solutions, LLC, a subsidiary of CCC, provides highly personalized care management that helps individuals achieve what matters most in their lives. 

 

The Connecticut Age Well Collaborative, an initiative of CCC, fosters inclusive, age-friendly communities across the state.

Michelle’s Story

Michelle lives with her mother, Peggy, and shares a warm, close-knit bond with her extended family, who remain an important part of her daily life. Surrounded by this steady foundation of love and support, Michelle flourishes in her community. One of her greatest passions is musical theatre. Each year, she proudly maintains her subscription to The Bushnell, where she attends shows from her reserved accessible seat—a place that feels like her own connection to the stage and the art form she loves.

Michelle’s sense of belonging stretches back to her childhood summers at Camp Harkness. The friendships she formed there have grown into lifelong relationships, with regular reunions that keep the spirit of camp alive year after year. For Michelle, these gatherings are more than social occasions—they are cherished traditions that remind her of the community she helped build and still treasures deeply.

Her personal commitments extend into advocacy as well. Michelle has become a recognized voice for disability rights, serving on multiple state commissions and advisory boards. In these roles, she offers not only her perspective but also her leadership, ensuring that the voices of people with disabilities are heard in shaping policies and opportunities.

When financial challenges once threatened to prevent her from attending Camp Harkness, Michelle’s network of family and supporters stepped in. Together, they worked to secure community-based grant funding, including a CCC Live Home Support donation. This collaboration ensured Michelle could continue participating in the camp experience that has brought her so much joy, empowerment, and connection throughout her life.
