We partner with the State Department of Social Services (DSS) to connect individuals to professional care management through Long-Term Services and Supports (LTSS) programs, all requiring qualification. In addition, Sage Solutions, a CCC subsidiary, provides private care management including daily support to help people continue to live at home, most often paid for privately or through long-term care insurance. We also offer specialized care transitions services from all care levels as part of the Connecticut Home Care Program.
We’re committed to helping more than 16,000 people live at home rather than be admitted to a nursing home or other institutional setting. Qualified individuals can access The Personal Care Assistance Medicaid Waiver Program which allows individuals with a physical disability that require hands-on help with at least two daily living activities—bathing, dressing, eating, transferring or toileting— to receive services. Even if a person is in a nursing home or long-term care facility, there is still the option to return home or to the community. Money Follows the Person (MFP) provides the funds, services, and support for a successful return.
