Programs

We partner with the State Department of Social Services (DSS) to connect individuals to professional care management through Long-Term Services and Supports (LTSS) programs, all requiring qualification.  In addition, Sage Solutions, a CCC subsidiary, provides private care management including daily support to help people continue to live at home, most often paid for privately or through long-term care insurance.  We also offer specialized care transitions services from all care levels as part of the Connecticut Home Care Program.

The Connecticut Home Care Program (CHCP)

We’re committed to helping more than 16,000 people live at home rather than be admitted to a nursing home or other institutional setting.  With guidance and helpful solutions, we work to make staying at home a reality whenever possible. Upon approval, we become partners, solution seekers, navigators, and advocates ready to make the process easier on the individual and family.

Community First Choice (CFC)

Qualified individuals can access Community First Choice, an optional State Plan service which allows eligible individuals to receive Personal Care Assistance (PCA) and other services and supports through self-direction. Upon approval, we become partners, solution seekers, navigators, and advocates that make the process easier for the individual and family.

Personal Care Assistance (PCA)

Qualified individuals can access The Personal Care Assistance Medicaid Waiver Program which allows individuals with a physical disability that require hands-on help with at least two daily living activities—bathing, dressing, eating, transferring or toileting— to receive services. Upon approval, we become partners, solution seekers, navigators, and advocates that make the process easier for the individual and family.

Money Follows the Person (MFP) Program

Even if a person is in a nursing home or long-term care facility, there is still the option to return home or to the community. Money Follows the Person (MFP) provides the funds, services, and support for a successful return.

The program is tailored to a person’s individual needs. Upon approval, we become partners, solution seekers, navigators, and advocates that make the transition easier for the individual and family.

Acquired Brain Injury Waivers (ABI)

If an individual is Medicaid-eligible and a Connecticut resident with an acquired brain injury and currently receives or requires services in an institutional setting, we provide a person-centered plan of care outlining an array of non-medical, home and community-based services. We become partners, solution seekers, navigators, and advocates that make the process easier on the individual and family.
