Provider Enrollments and Changes

New provider enrollments are not handled by CCC. Providers should enroll with both GT Independence and CT Department of Social Services.

Once enrolled the Provider will receive confirmation and be added to GT Independence Directory. Providers should then submit enrollment confirmation documentation to CCC online via this form.

For changes to existing provider information, GT Independence does not directly inform Access Agencies of Directory additions/changes. Providers should use this
form for all changes to existing provider information.
