Live Well and Live Well with Diabetes, evidence-based chronic disease self-management and diabetes self-management programs developed by Stanford University, helps individuals like you take charge of their lives by building self-management skills. Designed for those with ongoing health conditions, the programs encourage healthier behaviors, build self-confidence to improve communication with health providers and identify better ways to deal with pain, fatigue and stress. CCC, supported by the Connecticut State Department on Aging and Department of Public Health, trains and certifies workshop leaders and coordinates a broad array of community partners who recruit, deliver and host programs in local gathering places. Programs are delivered in English and Spanish.
