Innovation fuels our mission. Sage Solutions, a fast-growing CCC subsidiary, is a multidisciplinary team of professionals educated and trained in the field of geriatrics, private care management and patient advocacy, with Sage Navigators
guiding individuals toward receiving highly personalized care management.

The Connecticut Age Well Collaborative, a statewide, cross-sector part of CCC that fosters aging, dementia and disability-inclusive communities while building connectedness across communities — from celebrating towns advancing accessibility and inclusiveness through Wellspring Awards to Daring Dialogues on Ageism and Ableism in towns across the state, Age Well Academy sessions that highlight topics like the Connecticut Healthy Aging Report, Community Fellowship Grants, and even taking A Kids Book About Ageism on the road! Our people make our mission possible. A happy, healthy workplace is essential to sustaining the energy and passion that drive our work forward, which is why we prioritize workplace professional growth, well-being, and safety. This year, we launched Safe Practices, Strong Teams: Wellbeing in the Field and Beyond, a comprehensive workplace safety strategy that reached all of our employees—more than 300 staff—through targeted training and practical safety techniques. We foster a supportive environment where teamwork, balance, and open communication are valued, and we continually recognize outstanding team members whose dedication to the people we serve inspires us every day.
