This year’s Gratitude and Impact Report centers on the theme, “Together, We’re Uplifting Lives and Inspiring Communities.” With your support, we’re helping people thrive in their homes and communities. I’m proud to share several of this year’s milestones, including:
• Serving more than 16,000 people across our state,
• Acquiring a private care management business, helping us reach more people,
• Securing vital grants to enhance our community work and IT infrastructure,
• Building valuable new partnerships statewide, and
• Integrating value-based payment principles.
None of this progress would be possible without you. Your support and our partnership light the path to a hopeful future filled with potential. Together, we continue to uplift lives, inspire communities, and build on a legacy spanning more than 40 years. Thank you.

We are committed to advancing effective solutions and strategies, ensuring we deliver high-quality care at home and within our communities for future generations. Additionally, we’re broadening service options and expanding our presence in communities across the state, all while maintaining an unwavering focus on quality care management, increasing referrals, which have nearly doubled in the past year, and ensuring client satisfaction. Here’s what else we’ve been up to!
We’re hyper-focused on creating a workplace that’s committed to fostering stability, talent acquisition and advancing equity and belonging.
Sage Solutions is a CCC private care management and patient advocacy subsidiary that works with individuals and families with the goal of optimizing the quality of life and well-being of the older adult. The Sage approach leverages individual, customized care planning that is designed to meet the specific needs of the family, with high touch consulting, advocacy, and decades of experience in dementia care.
Every Age, Every Ability is a public outreach campaign launched by the Connecticut Age Well Collaborative, a CCC initiative that fosters aging, dementia and disability-inclusive communities.
Adding to its long list of accomplishments, Collaborative staff teamed up with community and Steering Committee leaders to write A Kids Book About Ageism. Written in a co-creative process and informed by lived experience, the book explores what ageism is and why it hurts us all.
In partnership with the State of Connecticut, we provide in-home care management and transitional support to thousands of people through these programs:
In addition, Sage Solutions, a CCC subsidiary, provides customized, private care management services including eldercare advice, consulting, dementia care management and patient advocacy to older people and their families.
And through The Connecticut Age Well Collaborative initiative we lead, collaborate and make connections that foster positive outcomes for people of all ages and abilities who live and work there.
Jaden was resuscitated twice on the way to the hospital. As his condition showed little improvement, Kathleen, Jaden’s CCC Care Manager, faced an immense challenge. “When I first assessed him, he was unresponsive, needed a ventilator to breathe, and required assistance due to his fragile airway,” she recalled.
Initially, plans were made to transfer Jaden to a nursing facility and halt therapy due to his lack of progress. However, his mother refused to accept this outcome and pleaded with his Care Manager to get him home.
Then something remarkable happened: Jaden’s eyes opened. This was a glimmer of hope. His mother’s efforts were paying off. Day by day, Jaden’s condition at the hospital steadily improved. He eventually was able to breathe on his own—a monumental step toward returning home. His mother began preparations, first moving to a first-floor apartment, and with guidance and support from the CCC Care Team made necessary home modifications, completed home care training, and arranged for Private Care Assistants.
Michelle, Jaden’s CCC Transition Coordinator, played a pivotal role in this journey. She coordinated home accessibility updates, navigated Medicaid benefits, and ensured that Jaden’s needs were met every step of the way.
Later, thanks to CCC donor generosity, Jaden’s journey was made a little easier by providing him with a cushion grip switch to activate his iPad, which helped him regain a sense of empowerment and normalcy in his daily life.
Today, Jaden is home, enveloped by the love of his family. The immense smile that illuminated his face upon returning home spoke volumes, sending a powerful message: “I’m home.”

Meir, who has familial dysautonomia, is among only 290 people worldwide with this disorder and is the second youngest known case globally. His fight began early; as an infant, he experienced a stroke and faced repeated hospitalizations due to the fragility of his lungs. Despite hurdles, Meir’s spirit remains unbroken.
For years, Meir’s father, a devoted Rabbi, shouldered the heavy financial burden of his care. The breakthrough came when the Money Follows the Person (MFP) Program, Department of Developmental Disabilities (DDS) waivers, and CCC together helped make Meir’s return home possible.
Michelle, Meir’s CCC MFP Transition Coordinator, reveals the profound impact Meir has on those around him. “Every nurse and caregiver who meets Meir falls in love with him,” she says. “His infectious smile and silent laugh—so powerful it shakes his whole body—make him an irreplaceable source of joy.”
Meir’s love for Judge Judy and fascination with zoo animals became the heart of his recent homecoming celebration. Staff and caregivers organized a heartwarming parade just for him: they donned zoo animal masks, and one dressed as Judge Judy, bringing a burst of color and joy to the hospital hallways.
His story is a celebration of resilience, a testament to the power of love, and a reminder of the incredible impact one person’s spirit can have on others.
