On any given day, our 300 plus care managers listen, assess, and help people achieve their goals by developing customized plans of care and delivering high-quality care management and transitional support. Our team shares information about critical resources, provides connection to quality services and supports and follow up with ongoing guidance and further engagement as life circumstances change. Through our national accreditations (National Committee for Quality Assurance (NCQA), CCC affirms its commitment to person-centered, quality care management.
In addition, Connecticut Community Care very much values our provider network. To guarantee fairness and equitable distribution of referrals, Connecticut Community Care utilizes the following methodology:
- Client choice is always a first option to consider.
- If a CCC client doesn’t have a preferred care services provider, a CCC Care Manager uses a specially designed, proprietary software system to ensure equitable randomization of referrals to a provider enrolled with the Department of Social Services that can render the required services.
